Overview

Electrolyte and fluid homeostasis refers to the complex physiological process of maintaining the balance of water and essential ions, such as sodium, potassium, chloride, and bicarbonate, within the body's compartments (Shrimanker & Bhattarai, 2023). This process is critical for cellular function, nerve conduction, muscle contraction, and the maintenance of blood pressure and volume (NIH/NIDDK, 2024). It is primarily regulated by the kidneys, the endocrine system—including hormones like antidiuretic hormone (ADH), aldosterone, and atrial natriuretic peptide—and the central nervous system (Lewis, 2023). While not a single molecular target itself, it is the physiological endpoint for numerous therapeutic interventions. Dysregulation of this balance leads to conditions such as hypertension, heart failure, and renal disease. Pharmacological agents like diuretics, vasopressin antagonists, and renin-angiotensin-aldosterone system (RAAS) inhibitors work by targeting specific molecular components within this homeostatic network to restore balance.
