Overview

Enamel matrix derivative (EMD) is a purified protein extract derived from developing porcine enamel matrix, primarily consisting of amelogenins (Gestrelius et al., 2000, PubMed). It is the active biological component in products like Emdogain, which are used in regenerative dentistry to treat periodontal defects and promote the restoration of lost tooth-supporting tissues (Miron et al., 2016, PubMed). EMD functions by mimicking the natural process of tooth development, specifically the role of the enamel matrix in inducing cementogenesis and the subsequent formation of the periodontal ligament and alveolar bone (Hammarström, 1997, PubMed). The clinical efficacy of EMD is heavily influenced by its ability to adsorb onto various surfaces, including root dentin and synthetic bone ceramics, where it serves as a bioactive scaffold for cell signaling and attachment (Miron et al., 2011, PubMed). Although EMD is a potent therapeutic agent, it is classified as a biologic drug rather than a traditional molecular target like a receptor or enzyme.

Mechanism of action

EMD mimics the biological environment of the developing tooth root to stimulate the formation of acellular extrinsic fiber cementum, which provides a foundation for new periodontal ligament and alveolar bone attachment (Hammarström, 1997, PubMed).
