Overview

Enamel matrix derivative (EMD) is a bioactive material composed of a suite of proteins, predominantly amelogenins, derived from developing porcine teeth that serves as a scaffold for periodontal regeneration (Gestrelius et al., 1997). It mimics the biological environment of the dental follicle during tooth development, facilitating the formation of a functional attachment apparatus including cementum, periodontal ligament, and alveolar bone. EMD functions by promoting the recruitment, attachment, and proliferation of mesenchymal cells while modulating the activity of endogenous growth factors such as Transforming Growth Factor-beta (TGF-beta) and Bone Morphogenetic Proteins (BMP) (Bosshardt, 2008). These interactions trigger intracellular signaling cascades, including the MAPK/ERK pathways, which lead to the expression of genes involved in wound healing and osteogenesis (Grandini et al., 2008). Clinically, EMD is applied as a gel (commercially known as Emdogain) to root surfaces during periodontal surgery to treat intrabony defects and gingival recessions. While highly effective in promoting biological regeneration, its use requires careful surgical technique and is primarily indicated for specific types of periodontal lesions.
