Endogenous dysfunctional T lymphocyte replacement refers to therapeutic strategies aimed at removing or replacing a patient’s own (endogenous) T cells that have become dysfunctional—often due to chronic disease, cancer, or exhaustion—and restoring immune competence by enabling the proliferation of functional effector and memory T cells. This approach is particularly relevant in the context of immunotherapy for cancer and autoimmune diseases. Strategies include physical removal, selective apoptosis, homeostatic proliferation, adoptive cell transfer and gene editing to remove/replace dysfunctional T cells and promote functional immune reconstitution.
