Endothelial dysfunction refers to an unhealthy or impaired functional state of the cells lining blood vessels—the endothelium. This condition is characterized by reduced vasodilation capacity due to decreased nitric oxide availability; increased vascular permeability; pro-thrombotic activity; heightened inflammation; and abnormal responses to physiological stimuli. It plays a central role in the pathogenesis and progression of cardiovascular diseases such as coronary artery disease, heart failure, hypertension, diabetes-related complications, and atherosclerosis. While drugs such as statins and ACE inhibitors can improve endothelial health through various mechanisms—including enhancing nitric oxide signaling—there is no single molecular entity called “endothelial function impairment” that serves as a direct drug target. Instead, it represents an integrated marker/state reflecting cumulative risk from multiple factors including oxidative stress, dyslipidemia, hypertension, metabolic disturbances, genetic predisposition—and thus requires multifaceted intervention strategies focused on both pharmacologic therapy and lifestyle modification for effective management.
