Overview

Endothelial function improvement" refers to therapeutic or lifestyle interventions aimed at restoring or enhancing the normal physiological functions of endothelial cells lining the blood vessels. Healthy endothelium is essential for regulating vascular tone, inhibiting inflammation, controlling blood fluidity, and maintaining vascular integrity. Endothelial dysfunction is associated with reduced nitric oxide availability, increased oxidative stress, vascular inflammation, greater risk of thrombosis, and is both a marker and driver of cardiovascular and metabolic diseases. Strategies for endothelial function improvement include pharmacological agents (such as statins, RAS inhibitors, and novel NO donors), non-pharmacological approaches (diet, exercise, stress reduction), and emerging molecular therapies targeting endothelial signaling pathways. While endothelial function is not itself a direct drug target, many therapies are evaluated for their impact on endothelial health as a key mechanism for cardiovascular risk reduction.

Mechanism of action

Enhancement of nitric oxide (NO) bioavailability and production. Suppression of endothelial inflammation (including KLF2/KLF4 upregulation). Reduction of oxidative stress and ROS. Modulation of PI3K/Akt signaling. Inhibition of pro-inflammatory cytokines and molecules (e.g., ICAM1, MCP1, VCAM1, IL6). Epigenetic modulation (e.g., by SIRT6).
