Overview

Under inflammatory conditions, mediators like thrombin or TNF-alpha trigger signaling cascades, such as RhoA and NF-kappaB, that lead to junctional disassembly and increased permeability, facilitating leukocyte extravasation (PubMed, 2023). Dysregulation of these processes is central to the pathogenesis of sepsis, acute respiratory distress syndrome (ARDS), and diabetic microvascular complications (StatPearls, 2024). Pharmacological intervention targets these pathways through various mechanisms, including S1P receptor modulation to stabilize junctions or VEGF inhibition to reduce pathological leakage (Pharmacological Reviews, 2021). Understanding these integrated pathways is crucial for developing therapies that restore vascular homeostasis and prevent organ edema.

Mechanism of action

Therapeutic agents modulate these pathways by stabilizing endothelial cell-cell junctions through S1P receptor agonism, inhibiting pro-inflammatory transcription factors like NF-kappaB, or blocking permeability-inducing growth factors such as VEGF and Angiopoietin-2 (NCBI, 2023; Nature Reviews Immunology, 2021).
