Neutral endopeptidase (NEP), also known as neprilysin or CD10, is a zinc-dependent metallopeptidase that plays a critical role in the degradation of a wide range of bioactive peptides, including atrial natriuretic peptide (ANP), brain natriuretic peptide (BNP), enkephalins, and substance P. It is a type II integral membrane protein with a short cytoplasmic domain, a transmembrane helix, and a large extracellular domain containing the catalytic site. Expressed in various tissues, including kidney, lung, heart, and brain, NEP regulates blood pressure, modulates nociceptive responses, and controls inflammatory processes. Inhibitors of NEP have been explored as potential treatments for hypertension and heart failure, although potential safety concerns exist due to its broad substrate specificity and potential for increasing levels of vasoconstrictors.
