Overview

Epidermal ceramide composition refers to the specific profile and concentration of various ceramide subclasses within the stratum corneum, which is the skin's primary physical barrier. Ceramides are sphingolipids composed of a sphingoid base linked to a fatty acid, and they represent nearly 50% of the total lipid content in the epidermal intercellular space. Their biological function is to form structured lamellar bilayers that prevent excessive transepidermal water loss (TEWL) and shield the body from external chemical and biological insults (Uchida & Park, 2021). Alterations in the ceramide composition—such as a reduction in long-chain ceramide subclasses—are hallmarks of inflammatory skin diseases like atopic dermatitis and psoriasis (Coderch et al., 2003). Therapeutically, this composition is targeted through the application of topical barrier repair formulations containing physiological lipids or via agents that stimulate endogenous ceramide production (Meckfessel & Brandt, 2014). Maintaining the correct balance of ceramides, cholesterol, and free fatty acids is essential for the restoration of the skin barrier in clinical dermatology.
