The epileptogenic zone network (EZN) refers to a distributed set of interconnected brain regions and circuits that are collectively necessary and sufficient for the generation and propagation of spontaneous seizures. Traditionally conceptualized as a focal 'trigger,' modern epileptology recognizes that seizures arise from abnormal dynamics within a large-scale network involving the seizure onset zone (SOZ), the irritative zone, and the propagation zone. While not a single molecular target like a protein or receptor, the EZN is the primary clinical target for neurosurgical interventions (resection or thermal ablation) and neuromodulation therapies such as Responsive Neurostimulation (RNS) and Deep Brain Stimulation (DBS). Pharmacological treatments aimed at the EZN involve anti-seizure medications that modulate neuronal excitability by targeting ion channels and neurotransmitter systems within these circuits to prevent the synchronization of pathological discharges. Identifying the EZN is critical for the management of drug-resistant epilepsy, requiring a multimodal approach that integrates stereoelectroencephalography (SEEG), high-frequency oscillations (HFOs), and advanced functional neuroimaging.
Drugs modulate the network by inhibiting voltage-gated sodium or calcium channels, enhancing GABAergic inhibitory neurotransmission, or antagonizing glutamatergic excitatory receptors. Surgical and neuromodulation approaches involve the physical removal or electrical inhibition of critical network nodes.
