Epithelial downgrowth refers to a rare but severe ophthalmologic complication where epithelial cells—normally found only on the surface of the eye—invade the intraocular space following surgery or penetrating eye injury. This can lead to progressive visual impairment due to proliferation and migration of these cells across intraocular surfaces such as the corneal endothelium or inside the anterior chamber, often forming sheets or membranes. Complications can include secondary glaucoma, corneal edema, inflammation, and eventual loss of vision or even the eye itself in severe cases. There are no known drugs that act on "epithelial downgrowth" as it is not a discrete molecule or pathway, but rather an abnormal healing response and cell migration event[1][3][5][7][9][10].
