The epithelial glycocalyx is a dense, carbohydrate-rich layer coating the apical surface of epithelial cells in organs such as the lungs, gut, and eyes (PMID: 30135137). Composed of proteoglycans, glycoproteins (like mucins), and glycolipids, it acts as a critical physical and chemical barrier that regulates molecular passage and protects cells from mechanical shear and pathogen invasion (PMID: 32654341). In conditions like acute respiratory distress syndrome (ARDS), sepsis, and cancer, the glycocalyx undergoes enzymatic degradation or shedding, which compromises barrier integrity and promotes inflammation (PMID: 31430275). Drugs targeting the glycocalyx aim to either inhibit degradative enzymes like heparanase and matrix metalloproteinases or replenish the layer using glycosaminoglycan mimetics (PMID: 33561561). Because it is the first point of contact for many pathogens and drugs, maintaining its integrity is essential for organ homeostasis and effective drug delivery (PMID: 30659018).
Therapeutic strategies focus on preventing the degradation of glycocalyx components by inhibiting shedding enzymes like heparanase and matrix metalloproteinases (MMPs), or by providing exogenous glycosaminoglycan mimetics and precursors to restore the structural integrity and barrier function of the layer (PMID: 33561561, PMID: 35588887).
