Overview

Epithelial sodium channel messenger RNA (ENaC mRNA) refers to the transcripts, primarily SCNN1A, SCNN1B, and SCNN1G, that encode the alpha, beta, and gamma subunits of the heterotrimeric ENaC protein. This channel is a key regulator of sodium and water homeostasis across tight epithelia in the lungs, kidneys, and colon. In the respiratory system, ENaC-mediated sodium reabsorption is a major determinant of airway surface liquid (ASL) volume and mucociliary clearance. In cystic fibrosis (CF), ENaC is often hyperactive, leading to dehydrated mucus and impaired lung defense, making its mRNA a strategic target for knockdown therapies. Drugs targeting ENaC mRNA, such as antisense oligonucleotides (ASOs) and siRNAs, aim to reduce the production of the channel protein to restore airway hydration and improve pulmonary function. While clinical trials have shown that these therapies can successfully reduce mRNA levels in the lung, some programs have faced challenges due to long-term preclinical toxicity concerns.
