Overview

Epstein–Barr virus nuclear antigen 1 (EBNA1) is a multifunctional viral protein essential for the replication, segregation, and maintenance of the EBV episomal genome during latent infection. It is uniquely expressed in all EBV-associated malignancies, including nasopharyngeal carcinoma and various lymphomas, as well as in cells associated with autoimmune conditions like multiple sclerosis. The therapeutic target consists of EBNA1-derived peptide fragments presented on the cell surface by Major Histocompatibility Complex (MHC) class I and II molecules. These peptide-MHC (pMHC) complexes serve as the primary recognition signal for the adaptive immune system, specifically for CD8+ and CD4+ T cells. Modern therapeutic approaches focus on enhancing this recognition through TCR-engineered T cells, therapeutic vaccines, and bispecific T-cell engagers to selectively eliminate EBV-positive cells. Despite the virus's attempts to limit EBNA1 processing through its Gly-Ala repeat domain, sufficient peptide presentation occurs to make these complexes viable targets for immunotherapy.

Mechanism of action

T-cell receptor (TCR) binding to the peptide-MHC complex triggers cytotoxic T-lymphocyte (CTL) mediated lysis of EBV-infected or transformed cells and stimulates helper T-cell cytokine production.
