Epstein–Barr virus (EBV) peptide–major histocompatibility complex (pMHC) refers to the molecular assembly of viral-derived peptides bound within the groove of MHC molecules on the surface of EBV-infected or malignant cells (Source: NCBI, PMC7066314). These complexes are essential for the adaptive immune system, specifically CD8+ cytotoxic T lymphocytes, to recognize and eliminate cells harboring the virus (Source: UniProt). In EBV-associated malignancies such as nasopharyngeal carcinoma and post-transplant lymphoproliferative disorder (PTLD), these pMHCs serve as highly specific tumor-associated antigens (Source: Journal of Hematology & Oncology, 2021). Therapeutic strategies targeting these complexes include adoptive T-cell therapies, such as tabelecleucel, and engineered T-cell receptor (TCR) therapies designed to bind specific EBV epitopes from proteins like LMP1, LMP2, or EBNA1 (Source: Atara Biotherapeutics; ClinicalTrials.gov). By focusing on these viral targets, therapies can achieve high specificity for diseased cells while sparing healthy, non-infected tissue (Source: Nature Reviews Cancer). However, therapeutic challenges include the potential for immune evasion through the downregulation of MHC molecules by the virus or tumor cells, as well as the requirement for HLA matching between the therapy and the patient (Source: PubMed; EMA).
