The Epstein-Barr virus (EBV) peptide-Human Leukocyte Antigen (HLA) class I complex is a molecular assembly presented on the surface of cells infected or transformed by EBV. It consists of a viral peptide fragment, often derived from latent membrane proteins (LMP1, LMP2) or nuclear antigens (EBNA), bound within the groove of an HLA class I molecule. This complex acts as the specific ligand for the T-cell receptors (TCRs) of CD8+ cytotoxic T-lymphocytes, facilitating the immune system's ability to identify and eliminate virally infected cells. In EBV-associated cancers, such as post-transplant lymphoproliferative disorder (PTLD) and nasopharyngeal carcinoma, these complexes are exploited as therapeutic targets for adoptive T-cell therapies and TCR-engineered cells. Drugs like tabelecleucel utilize this mechanism by providing EBV-specific T-cells that recognize these complexes to induce targeted cell lysis. The therapeutic efficacy is highly dependent on the match between the drug's TCR specificity and the patient's HLA allele, making HLA typing a critical component of clinical application.
