The erythrocyte membrane-cytoskeleton complex is a specialized structural network that determines the shape, stability, and deformability of red blood cells (RBCs). It consists of a lipid bilayer anchored to a two-dimensional protein meshwork primarily composed of spectrin, actin, ankyrin, and protein 4.1, which interact with integral membrane proteins like Band 3 and glycophorins (Mohandas & Gallagher, 2008). This organization allows the RBC to withstand high shear stress and undergo the extreme deformations required to navigate narrow capillaries and splenic slits (Da Costa et al., 2013). In diseases such as sickle cell anemia, malaria, and diabetes, the membrane becomes rigid due to oxidative damage, protein cross-linking, or pathological hemoglobin interactions, leading to impaired microcirculation and hemolysis (Vichinsky et al., 2019). Pharmacological agents like pentoxifylline and voxelotor target these components or their environment to improve rheological properties, reduce blood viscosity, and enhance oxygen delivery to tissues (Diederich et al., 2018).
Increasing intracellular ATP levels to maintain active transport, reducing oxidative stress on structural proteins, inhibiting pathological hemoglobin polymerization, and modulating lipid bilayer fluidity.
