Escherichia coli colonization factor antigen CS2 is a fimbrial adhesin (pili/fimbria) found on the surface of enterotoxigenic Escherichia coli (ETEC), which are a common cause of childhood and traveler’s diarrhea[1][2][3][4]. CS2 is part of the CFA/II (colonization factor antigen II) family and functions as a major virulence factor, mediating the initial adhesion of ETEC to the epithelial cells lining the small intestine[3][4]. Structurally, CS2 forms rigid, helix-like pili composed primarily of the major pilin subunit CotA and additional minor subunits (CotB, CotC, CotD)[2][3]. The genes encoding CS2 are located in a cluster (cotA, cotB, cotC, cotD) and are regulated both by chromosomal elements and by a plasmid-encoded positive regulator, rns[2][4]. CS2 fimbriae are immunogenic, stimulate a protective immune response, and are targets of ongoing vaccine research[1][3]. Blocking CS2 function by vaccination or by monoclonal antibodies can prevent ETEC colonization, reduce diarrheal incidence, and promote protective immunity in animal models and potentially humans[1][3]. No drugs target CS2 directly; however, its presence is used as a biomarker for ETEC in epidemiological surveillance and candidate vaccines often include CS2 as a key protective antigen[1][3]. The main therapeutic challenge is the polymorphism of colonization factors among ETEC strains, requiring vaccines to cover multiple antigens for broad efficacy[1][3].
Antibodies or vaccine-elicited immune responses can prevent bacterial adhesion to epithelial cells by binding to CS2 fimbriae, blocking colonization and subsequent disease. No small-molecule inhibitors or direct antimicrobials are established.
