Overview

The esophageal and gastroesophageal junction (GEJ) mucosal surface is the epithelial lining that serves as a critical barrier between the lumen of the esophagus and the underlying tissue (StatPearls, 2023). It consists of stratified squamous epithelium in the esophagus, which transitions to columnar epithelium at the GEJ, often referred to as the Z-line (NIH, 2024). This surface is frequently the site of injury in conditions like gastroesophageal reflux disease (GERD) and esophagitis, where acid and pepsin cause inflammation and erosion (Mayo Clinic, 2023). While not a single molecular target, it is the site of action for mucosal protectants like sucralfate and alginates, which form physical barriers to prevent further damage (PubMed, 2022). Chronic irritation of this surface can lead to metaplastic changes, such as Barrett's esophagus, increasing the risk of adenocarcinoma (NCI, 2023). Therapeutic interventions often focus on reducing the acidity of the luminal contents or providing a mechanical shield to allow the mucosal surface to heal. Understanding the integrity of this surface is vital for managing esophageal disorders and preventing progression to malignancy.
