Overview

The esophageal epithelial luminal surface is the innermost lining of the esophagus, primarily composed of non-keratinized stratified squamous epithelium that serves as a critical physical and chemical barrier (StatPearls, 2023). This surface protects underlying tissues from mechanical trauma during swallowing and from chemical injury caused by refluxed gastric acid and pepsin (PubMed, PMID: 24533239). In pathological states such as gastroesophageal reflux disease (GERD) and eosinophilic esophagitis (EoE), the integrity of this surface is compromised, often characterized by dilated intercellular spaces and inflammatory cell infiltration (NIH, 2022). While not a specific molecular target like a receptor or enzyme, it is the primary site for topical drug delivery, where agents like budesonide or mucosal protectants (e.g., sucralfate) are applied to exert local effects (PubMed, PMID: 31210151). Therapeutic strategies often focus on enhancing this barrier or delivering anti-inflammatory agents directly to the luminal surface to minimize systemic side effects.
