The esophageal mucosal surface and luminal irritants represent the physiological and pathological interface involved in esophageal health and disease (StatPearls, 2023). The esophageal mucosa serves as a critical barrier against luminal irritants such as gastric acid, pepsin, and bile salts, which can reflux from the stomach (Nature Reviews Disease Primers, 2017). This defense system is organized into pre-epithelial, epithelial, and post-epithelial layers that maintain tissue integrity (PubMed, 2000). When the balance between these protective factors and the aggressive luminal irritants is disrupted, it results in mucosal injury, inflammation, and symptoms of gastroesophageal reflux disease (GERD) (NIH, 2020). Pharmacological management focuses on either reducing the acidity and volume of the luminal irritants or providing a physical coating to reinforce the mucosal surface (PubChem, 2024). Understanding this interface is essential for developing treatments that prevent the progression of esophagitis to more severe conditions like Barrett's esophagus (Mayo Clinic, 2023).
