It is predominantly expressed in the pancreas, brain, and adrenal glands, where it regulates vital processes such as glucose-stimulated insulin secretion and neurotransmitter release (PubMed: 11884611). EPAC2 functions by activating small GTPases Rap1 and Rap2, which subsequently modulate cytoskeletal dynamics and exocytotic machinery (PubMed: 12606328). In clinical contexts, EPAC2 is a significant target for type 2 diabetes research because it mediates the effects of incretin hormones like GLP-1 on insulin release (PubMed: 21964404). Mechanism of action

EPAC2 acts as a guanine nucleotide exchange factor (GEF) for Rap1 and Rap2 GTPases, facilitating the exchange of GDP for GTP upon binding of cAMP (UniProt Q9Y266). This activation triggers downstream signaling cascades that regulate exocytosis, particularly insulin granule release in pancreatic beta cells via interactions with Rim2 and Piccolo, and modulates synaptic strength in the central nervous system (PubMed: 11884611, PubMed: 18463636).
