Exposed alpha- and epsilon-amino groups on tissue and plasma proteins represent the primary chemical targets for various surgical sealants and cross-linking agents (FDA, BioGlue SSED, 2001). The alpha-amino group is located at the N-terminus of polypeptide chains, while the epsilon-amino group is found on the side chain of lysine residues (PubChem, Glutaraldehyde, 2024). In a clinical setting, drugs such as glutaraldehyde react with these nucleophilic groups to form covalent bonds, effectively gluing tissues together or sealing leaks in blood vessels and organs (Journal of Biomedical Materials Research, 2005). This mechanism is widely utilized in cardiovascular and thoracic surgeries to achieve immediate hemostasis and structural reinforcement. Because these amino groups are ubiquitous on almost all proteins, the interaction is non-specific and occurs rapidly upon contact at the application site. While highly effective for mechanical sealing, the reactivity can also lead to localized cytotoxicity or the formation of unwanted adhesions if the agent spreads beyond the intended area (StatPearls, Surgical Sealants, 2023).
