Exposed dentin surface refers to the clinical condition where the protective layers of a tooth, such as enamel or cementum, are lost, leaving the underlying dentin and its microscopic tubules open to the oral environment (StatPearls, 2023). This exposure is the physiological basis for dentin hypersensitivity, as it allows external thermal, chemical, or mechanical stimuli to induce fluid flow within the tubules, which in turn activates nociceptors in the dental pulp (NIH/NIDCR, 2022). While not a traditional molecular target like a protein or enzyme, the exposed dentin surface is the primary site of action for various desensitizing pharmacological agents. These treatments aim to either physically seal the tubule orifices using mineralizing agents like calcium sodium phosphosilicate and fluorides or to reduce nerve excitability using potassium salts (PubMed, 2010). Effective management of this surface is critical for alleviating chronic dental pain and preventing further structural degradation of the tooth matrix.
Therapeutic agents act on the exposed dentin surface through two primary mechanisms: physical occlusion of the dentinal tubules to block fluid movement (Brännström's hydrodynamic theory) or chemical desensitization of the intradental nerves by increasing extracellular potassium ion concentration to inhibit repolarization and signal transmission (StatPearls, 2023; Journal of Conservative Dentistry, 2010).
