Extended depth-of-focus intraocular lenses (EDOF IOLs) using echelette diffractive optics are advanced medical devices designed to improve postoperative vision in cataract and presbyopia patients by elongating the focal range in the eye, providing better distance and intermediate vision without discrete multifocal points. The echelette diffractive design forms a step structure whose height, spacing, and orientation modify the diffraction of light. A well-known example is the Tecnis Symfony IOL, which combines diffractive achromatic technology (reducing chromatic aberration) and a proprietary echelette structure. Unlike traditional multifocal IOLs, which split light into two or three foci (leading to secondary out-of-focus images and visual disturbances like halos), echelette EDOF lenses aim to provide sharp images at multiple distances with fewer photic phenomena. This term does not correspond to a biological target for drug action. Instead, it represents an optical engineering advancement in intraocular lens design used for correcting presbyopia and post-cataract vision challenges.
