Extracellular calcium–phosphate equilibrium refers to the tightly regulated physiological process that maintains stable concentrations of ionized calcium and inorganic phosphate in the blood and extracellular fluid (StatPearls, 2023). This homeostasis is critical for skeletal integrity, nerve conduction, muscle contraction, and cellular metabolism (NIH, 2022). It is primarily governed by a complex feedback loop involving the parathyroid glands, kidneys, bone, and intestines, mediated by hormones such as parathyroid hormone (PTH), calcitriol (active vitamin D), and fibroblast growth factor 23 (FGF23) (PubMed, 2021). Dysregulation of this equilibrium is a hallmark of chronic kidney disease-mineral and bone disorder (CKD-MBD), leading to complications like vascular calcification and metabolic bone disease (Journal of Nephrology, 2020). While the equilibrium itself is a physiological state rather than a single protein, it is managed pharmacologically by targeting specific receptors and transporters like the calcium-sensing receptor (CaSR) and sodium-phosphate cotransporters (Nature Reviews Nephrology, 2019).
