Overview

The extracellular fluid volume (ECFV) compartment encompasses all body fluid outside of cells, primarily consisting of interstitial fluid and blood plasma (StatPearls, 2023). It plays a vital role in maintaining hemodynamic stability, transporting nutrients and oxygen to tissues, and removing metabolic waste products (Guyton and Hall, 2020). The volume and composition of the ECFV are tightly regulated by the kidneys through the actions of hormones such as aldosterone, antidiuretic hormone (ADH), and natriuretic peptides (NIH, 2022). While not a single molecular target, the ECFV is a critical physiological parameter manipulated by various drug classes, including diuretics which reduce volume by promoting sodium and water excretion, and intravenous fluids used for volume expansion (Merck Manual, 2023). Pathological expansion of the ECFV leads to clinical edema and is a hallmark of heart failure and renal disease, whereas ECFV depletion results in dehydration and potential circulatory collapse. Consequently, pharmacological management of the ECFV is essential for treating hypertension, congestive heart failure, and shock (PubMed, 2021).
