The extracellular matrix (ECM) and wound bed microenvironment constitute a complex biological system providing structural scaffolding and biochemical cues necessary for tissue regeneration (StatPearls, 2023). The ECM is composed of fibrous proteins like collagen and elastin, alongside glycosaminoglycans and proteoglycans, which collectively regulate cell adhesion and signaling (NCBI, 2022). In chronic wounds, this microenvironment is often characterized by excessive proteolysis, hypoxia, and a persistent inflammatory state that prevents normal healing (PubMed, 2021). Therapeutic strategies target this environment through wound bed preparation, utilizing enzymatic debriders like collagenase to remove non-viable tissue or advanced dressings to modulate moisture and pH (Journal of Wound Care, 2020). Additionally, recombinant growth factors such as becaplermin are applied to stimulate cellular proliferation within the matrix (FDA, 2022). Effective management of the wound bed microenvironment is essential for transitioning stalled wounds into a productive healing phase and preventing complications like fibrosis or infection.
Modulation of the wound environment through enzymatic debridement of necrotic tissue, restoration of structural integrity with scaffold substitutes, and biochemical stimulation of cellular pathways to promote the transition from inflammation to tissue remodeling.
