Extracellular matrix (ECM) components and water molecules constitute the non-cellular environment of tissues, providing essential structural support and biochemical signaling cues (NCI, 2023). The ECM is composed of fibrous proteins like collagen and elastin, as well as glycosaminoglycans and proteoglycans that sequester water to maintain tissue hydration and turgor (StatPearls, 2023). Water molecules within this matrix are critical for maintaining osmotic balance, nutrient transport, and lubrication throughout the body. In a therapeutic context, these elements are targeted by agents that modify the physical properties of a tissue or lumen. For example, osmotic laxatives like polyethylene glycol interact with water molecules to increase intraluminal volume and facilitate bowel movements (Mayo Clinic, 2023). Additionally, exogenous ECM components like hyaluronic acid are used as fillers or viscosupplements to restore structural integrity or lubrication in joints and skin (PubMed, 2022). Other agents, such as bulk-forming laxatives, interact with the aqueous environment to alter the rheology of the extracellular space. Because this target encompasses a vast array of molecules and physical states, it is generally considered a physiological system or structural environment rather than a discrete molecular receptor.
