Overview

The extracellular matrix glycosaminoglycan (GAG) layer is a specialized network of long, unbranched polysaccharides that coat cell surfaces and fill the interstitial spaces of connective tissues. Composed of repeating disaccharide units such as hyaluronan, heparan sulfate, and chondroitin sulfate, this layer is essential for maintaining tissue hydration, providing mechanical cushioning, and acting as a scaffold for cell-matrix interactions (StatPearls, 2023). In the urinary bladder, the GAG layer serves as a critical semi-permeable barrier that prevents toxic urinary constituents from penetrating the underlying epithelium and causing inflammation (Parsons, 2007). Pathological degradation or deficiency of this layer is implicated in conditions like interstitial cystitis and osteoarthritis, where the loss of structural integrity leads to pain and tissue damage (PubMed, 2021). Therapeutic strategies often involve the administration of exogenous GAGs or synthetic mimetics, such as pentosan polysulfate or hyaluronic acid, to replenish the layer and restore its protective or lubricative functions (FDA, 2020). Furthermore, GAGs interact with various growth factors and cytokines, making them pivotal regulators of cell signaling and inflammatory responses in cancer and wound healing (Nature Reviews Molecular Cell Biology, 2022).
