Overview

Extracellular matrix modulation refers broadly to altering the composition, structure, degradation rate, or signaling properties of the **extracellular matrix (ECM)**—the complex network surrounding cells in all tissues. The ECM consists mainly of proteins such as collagens and elastins; glycoproteins like fibronectin and laminins; proteoglycans; glycosaminoglycans; as well as associated enzymes including proteases. The dynamic state (“modulation”) can involve synthesis/deposition by stromal cells or degradation/remodeling by enzymes such as **matrix metalloproteinases**, often regulated during development, wound repair, inflammation/fibrosis processes—and pathologically during cancer invasion/metastasis. While individual molecules within this system can be therapeutic targets—for example specific integrins or MMPs—“extracellular matrix modulation” itself is not a discrete molecular entity but rather describes an entire biological process involving many targets across several molecular classes.[1][2][3][5] Because it does not refer to one specific molecule/receptor/protein but instead encompasses numerous pathways/processes involved in maintaining and remodeling tissue architecture at large scale,[3] “extracellular matrix modulation” should not be considered a canonical drug target per se. Instead it represents an important therapeutic strategy area relevant across multiple diseases where abnormal tissue structure contributes directly to pathology.[4]

Mechanism of action

Mechanisms for drugs that modulate the extracellular matrix include: Inhibition of proteases that degrade ECM proteins (e.g., MMP inhibitors); Blocking integrin-mediated cell adhesion to the ECM; Modifying cross-linking enzymes to alter tissue stiffness/elasticity. Again, these mechanisms are diverse because "extracellular matrix modulation" refers to many possible interventions.
