Extracellular matrix protein 1 (ECM1) is a secreted glycoprotein found in the extracellular matrix of most tissues and exists in multiple isoforms arising from alternative splicing (notably ECM1a, ECM1b, ECM1c, and ECM1d). ECM1 plays an essential role in structural support, modulation of cell growth and differentiation, angiogenesis, and maintenance of skin integrity, and it interacts with a variety of structural extracellular proteins. Mutations in ECM1 lead to lipoid proteinosis, a rare genetic disorder characterized by skin and mucosal abnormalities, while overexpression—particularly of the ECM1a isoform—promotes tumorigenesis and therapy resistance in multiple types of cancer, partly through integrin binding and activation of downstream AKT/FAK signaling pathways. ECM1 is a focus of emerging research in oncology for its value as a potential biomarker and as a target for anti-cancer therapy, though direct targeting agents are not yet in clinical use.
