Overview

The extracellular matrix (ECM) of human skin is a complex, three-dimensional network of macromolecules that provides essential structural support and biochemical signaling to the dermis and epidermis (StatPearls, 2023). It is primarily composed of fibrous proteins, most notably Type I and Type III collagen, which provide tensile strength, and elastin, which allows the skin to return to its original shape after stretching (NCBI, 2021). The ECM also contains a ground substance of glycosaminoglycans, such as hyaluronic acid, and proteoglycans like decorin, which are vital for maintaining tissue hydration and regulating the bioavailability of growth factors (PubMed, 2019). This matrix is not a static scaffold but a dynamic environment where components are constantly remodeled by enzymes such as matrix metalloproteinases (MMPs) and their inhibitors (UniProt, 2024). Dysregulation of skin ECM structural components is central to the pathology of photoaging, characterized by collagen fragmentation and solar elastosis, as well as genetic disorders like Ehlers-Danlos syndrome (NIH, 2022). Therapeutic interventions often target the ECM by using retinoids to stimulate collagen synthesis or using exogenous fillers and enzymes to modify matrix composition for aesthetic or medical purposes (Journal of Investigative Dermatology, 2020).
