The extracellular matrix (ECM) water and structural components constitute the complex, non-cellular environment that surrounds and supports cells within tissues (Alberts et al., 2002). This target includes the interstitial fluid and a scaffold of fibrous proteins and glycosaminoglycans, such as hyaluronic acid, that provide mechanical strength and regulate hydration (Papakonstantinou et al., 2012). In clinical practice, this target is addressed by agents that do not bind to specific receptors but instead modify the physical properties of the tissue environment (StatPearls, 2023). For example, ophthalmic lubricants restore the tear film's structural integrity, while osmotic diuretics and laxatives manipulate water distribution within the ECM to achieve therapeutic effects (Jones et al., 2017). Understanding the ECM is crucial for treating conditions ranging from dry eye syndrome and skin aging to cerebral edema and constipation (DrugBank, 2023).
Therapeutic agents targeting these components primarily function through physical or osmotic mechanisms. Humectants and lubricants supplement the structural components to retain moisture and reduce friction, while osmotic agents alter the water potential to induce fluid shifts across membranes or within the tissue matrix, thereby managing fluid volume and pressure.
