Forkhead box protein D4 (FOXD4) is a transcription factor in the forkhead/winged helix-box (FOX) family, characterized by a highly conserved DNA-binding domain. It regulates key developmental processes such as neural specification, proliferation, and metabolism. FOXD4 can repress differentiation-promoting genes and activate those supporting neural precursor renewal, thus biasing embryonic development toward neural fate. It is essential for normal pluripotency and has functional overlap with Oct4 in inducing stem cell-like states. Mutations (e.g., W148R) in FOXD4 are linked to complex syndromes combining cardiomyopathy and neuropsychiatric symptoms. Dysregulation is implicated in colorectal cancer progression (promoting EMT and metastasis) and is differentially expressed in leukemia and myeloproliferative neoplasms. Additional associations include congenital malformations and sex differentiation. FOXD4 is evolutionarily dynamic due to its location in subtelomeric regions, contributing to its transcriptional diversity and disease associations. No direct drugs target FOXD4, but expression and phosphorylation status may be modulated by upstream drug perturbations.
