The FXN regulatory DNA locus, situated on chromosome 9q21.11, is the genomic region responsible for the expression of frataxin, a protein vital for mitochondrial iron-sulfur cluster assembly (NCBI Gene ID: 2395). Friedreich's ataxia (FRDA) is primarily caused by an unstable GAA trinucleotide repeat expansion within the first intron of this locus, which induces epigenetic silencing and reduces frataxin production (Campuzano et al., 1996, Science). This deficiency leads to mitochondrial iron accumulation, increased oxidative stress, and the progressive loss of sensory neurons and cardiomyocytes. Therapeutic strategies targeting the FXN locus aim to restore frataxin levels by either excising the GAA repeats using gene-editing tools like CRISPR/Cas9 or by reversing the repressive chromatin state using histone deacetylase (HDAC) inhibitors (Gottesfeld et al., 2013, JBC). Additionally, synthetic transcription elongation factors (Syn-TEFs) are being developed to specifically bypass the transcriptional block created by the GAA repeats (Ouellet et al., 2017, Sci Rep). Successfully modulating this locus represents a curative approach to FRDA by addressing the root genetic cause of the disease.
