Free aldehyde groups in bovine pericardial tissue are reactive chemical residuals that remain after the glutaraldehyde cross-linking process used to manufacture bioprosthetic heart valves. While glutaraldehyde is necessary to provide mechanical strength and reduce the immunogenicity of the xenograft tissue, the unreacted or pendant aldehyde groups are inherently toxic and promote dystrophic calcification. These groups act as nucleation sites for calcium phosphate deposition, leading to the stiffening and eventual mechanical failure of the valve leaflets. To mitigate these effects, various post-fixation treatments are employed to 'cap' or neutralize these aldehydes using amino acids or reducing agents. Targeting these free aldehydes is a primary strategy in cardiovascular engineering to improve the biocompatibility and durability of tissue-based medical devices.
