Overview

Free heme refers to the iron-containing porphyrin molecule (iron protoporphyrin IX) that is not bound within hemoproteins. In its unbound state, free heme is highly reactive and toxic due to its ability to catalyze the formation of reactive oxygen species (ROS) and activate inflammatory pathways. Elevated levels are observed in hemolytic diseases, transfusion reactions, and tissue injury, contributing to organ dysfunction. Detoxification mechanisms include binding by plasma proteins, cellular export, and enzymatic degradation.

Mechanism of action

Many therapeutic strategies aim to neutralize or remove free heme, rather than directly interact with it in a traditional drug-target sense. These include enhancing heme scavenging by haptoglobin/hemopexin, promoting heme degradation via heme oxygenase, or inhibiting the inflammatory signaling pathways activated by heme.
