Overview

Free ionized calcium (Ca²⁺) is the physiologically active fraction of calcium in blood and tissues, constituting roughly half of serum calcium; it is not bound to proteins and is available to participate directly in vital biological processes. Free ionized calcium is essential for muscle contraction, blood coagulation, neural transmission, bone mineralization, signal transduction, and hormone secretion. Its homeostasis is tightly regulated by hormones, especially parathyroid hormone and vitamin D. Deviations from normal ionized calcium levels can result in serious clinical consequences, including tetany, arrhythmias, neurological dysfunction, and tissue calcification. Ionized calcium is measured directly in blood tests to diagnose and monitor conditions affecting calcium metabolism.
