Fusarium is a diverse and widely distributed genus of filamentous fungi within the phylum Ascomycota, encompassing species that are significant pathogens for both plants and humans. In agricultural contexts, these fungi cause devastating crop diseases and produce harmful mycotoxins like fumonisins and trichothecenes, which can contaminate food supplies and cause acute or chronic toxicosis. In clinical medicine, Fusarium species cause opportunistic infections known as fusariosis, ranging from superficial conditions like keratitis and onychomycosis in immunocompetent hosts to severe, often fatal disseminated infections in immunocompromised individuals. Unlike many other pathogenic fungi, members of the Fusarium genus are characterized by high levels of intrinsic resistance to multiple classes of antifungal drugs, including most azoles and echinocandins. Current therapeutic strategies rely primarily on high-dose liposomal amphotericin B and the second-generation triazole voriconazole, which target fungal cell membrane components or sterol biosynthesis pathways. Despite treatment, mortality rates for invasive fusariosis remain exceedingly high, particularly in patients with persistent neutropenia, making the management of these infections a major clinical challenge.
