Galactomannans are a diverse class of high-molecular-weight polysaccharides composed of a beta-(1,4)-D-mannopyranose backbone with alpha-(1,6)-D-galactopyranose side branches, found abundantly in the seeds of leguminous plants and the cell walls of fungi such as Aspergillus [1, 2]. In the pharmaceutical and food industries, they are widely utilized as gums (e.g., guar gum and locust bean gum) to serve as thickening agents, stabilizers, and dietary fiber supplements [3, 7]. Biologically, fungal galactomannan is a critical structural molecule that is released into host circulation during invasive growth, making it an essential biomarker for the early diagnosis and therapeutic monitoring of invasive aspergillosis [9, 11]. When administered as a therapeutic agent for metabolic health, galactomannans create a viscous environment in the gut that slows the absorption of carbohydrates and fats, which is beneficial for managing diabetes and hyperlipidemia [15, 19]. However, this high viscosity can also impede the absorption of essential drugs like digoxin or metformin, requiring careful clinical management of drug timing [21]. Furthermore, clinicians must account for potential diagnostic interference, as certain antibiotics and intravenous fluids have been documented to cause false-positive results in galactomannan-based immunoassay tests [3, 22].
In therapeutic use as a dietary fiber, galactomannans form highly viscous gels in the gastrointestinal tract that physically sequester nutrients and bile acids, thereby slowing gastric emptying and delaying the diffusion of glucose and lipids across the intestinal mucosa [15, 19]. In a diagnostic context, fungal galactomannans act as antigens characterized by beta-(1,5)-galactofuranosyl side chains that are specifically recognized by monoclonal antibodies in enzyme-linked immunosorbent assays (ELISA) to detect invasive fungal growth [2, 11].
