Gardnerella species, primarily Gardnerella vaginalis, are Gram-variable, facultative anaerobic bacteria that are central to the pathogenesis of bacterial vaginosis (BV) [StatPearls, 2023; Nature Reviews Microbiology, 2011]. These bacteria are known for their ability to adhere to vaginal epithelial cells, forming characteristic "clue cells" and developing robust, multi-species biofilms that protect them from host immune responses and antibiotic penetration [Nature Reviews Microbiology, 2011; BMC Microbiology, 2015]. Gardnerella species produce several virulence factors, including vaginolysin, a cholesterol-dependent cytolysin that causes host cell lysis, and sialidases that degrade the protective mucus layer of the vaginal tract [Journal of Infectious Diseases, 2005; Clinical Microbiology Reviews, 2019]. In clinical practice, these organisms are the primary targets of antimicrobial therapy using nitroimidazoles like metronidazole or lincosamides like clindamycin [CDC, 2021; StatPearls, 2023]. However, the persistence of Gardnerella within biofilms often leads to high rates of treatment failure and recurrence, posing a significant challenge in women's health [Nature Reviews Microbiology, 2011; Clinical Microbiology Reviews, 2019]. Emerging research also explores the role of Gardnerella in increasing susceptibility to other sexually transmitted infections, including HIV [Nature Reviews Microbiology, 2011].
Inhibition of DNA synthesis via reductive activation of nitro groups (nitroimidazoles) or inhibition of protein synthesis by binding to the 50S ribosomal subunit (lincosamides).
