Gardnerella vaginalis biofilm formation is a complex multicellular process that serves as the primary virulence mechanism in bacterial vaginosis (BV). The process begins with the adhesion of G. vaginalis to vaginal epithelial cells, facilitated by various adhesins and the toxin vaginolysin, leading to the formation of characteristic clue cells (Swidsinski et al., 2005). Once established, the biofilm produces an extracellular polymeric substance (EPS) matrix that provides a physical barrier against host immune defenses and increases the minimum inhibitory concentration (MIC) of antibiotics by up to 1000-fold compared to planktonic cells (Machado et al., 2016). This structural integrity is a major cause of treatment failure and high recurrence rates associated with standard therapies like metronidazole. Current research focuses on multi-modal approaches, including biofilm-disrupting agents like boric acid or DNase, and the use of probiotics to restore a healthy vaginal microbiome (Muzny et al., 2019). Targeting the biofilm is essential for achieving long-term clinical cure and preventing complications such as preterm birth and increased susceptibility to sexually transmitted infections (Alves et al., 2014).
