Gas bubbles in the gastrointestinal lumen are a physiological byproduct of swallowed air and the metabolic activity of gut microbiota during the fermentation of carbohydrates (NIH, 2023). While gas is a normal component of the digestive tract, its entrapment in the form of small, mucus-coated bubbles can lead to clinical symptoms such as bloating, abdominal distension, and flatulence (StatPearls, 2023). These bubbles often form a stable foam that resists expulsion, contributing to the discomfort associated with functional dyspepsia and irritable bowel syndrome (Mayo Clinic, 2022). Therapeutic intervention targets the physical properties of these bubbles rather than a specific biological receptor or enzyme. Antifoaming agents, most notably simethicone, work by reducing the surface tension of the gas bubbles, causing them to coalesce into larger bubbles that are more easily eliminated through belching or flatus (PubChem, 2024). This physical mechanism of action allows for the relief of pressure and pain without systemic absorption of the drug. Management of this target is essential in both chronic conditions like IBS and acute settings such as post-operative recovery or diagnostic preparation (PubMed, 2021).
