Overview

Gastric acid and bulk stomach contents represent the chemical and physical environment of the stomach lumen, primarily characterized by a high concentration of hydrochloric acid (HCl) and digestive enzymes (StatPearls, 2023). This environment is essential for the initial stages of digestion, specifically the denaturation of proteins and the activation of pepsinogen into the active protease pepsin (NIH, 2023). Additionally, the low pH serves as a critical innate immune barrier by neutralizing ingested pathogens before they reach the lower gastrointestinal tract (StatPearls, 2023). In conditions like gastroesophageal reflux disease (GERD) or peptic ulcers, the corrosive nature of these contents causes tissue damage and pain when the protective mucosal barriers are overwhelmed or when contents reflux into the esophagus (NIDDK, 2023). Unlike traditional molecular targets such as receptors or enzymes, gastric acid is targeted through direct chemical neutralization by antacids or physical sequestration by alginates, which form a protective raft on the surface of the bulk contents (PubMed, 2000).

Mechanism of action

Antacids neutralize hydrochloric acid through a chemical reaction to increase gastric pH (StatPearls, 2023). Alginates react with gastric acid to form a physical gel raft that floats on the stomach contents to prevent reflux (PubMed, 2000). Simethicone reduces the surface tension of gas bubbles within the bulk contents to facilitate their expulsion (StatPearls, 2023).
