The gastric acid pool and gastric contents refer to the complex mixture of hydrochloric acid, water, electrolytes, and digestive enzymes like pepsin found within the stomach lumen. This environment is physiologically essential for the denaturation of dietary proteins, the conversion of pepsinogen to its active form, and the destruction of ingested pathogens (StatPearls, NBK526049). However, when the protective mucosal barrier is compromised or when acid refluxes into the esophagus, these contents become the primary drivers of tissue damage in conditions such as peptic ulcer disease and gastroesophageal reflux disease (GERD) (NIH, MedlinePlus). Unlike most therapeutic targets that are specific proteins, the gastric acid pool is a physiological target for drugs that act through local chemical or physical mechanisms. Antacids interact with this target by directly neutralizing the acid to raise the pH, while alginates react with the acid to form a physical 'raft' that floats atop the gastric contents to block reflux (DrugBank, T112; PubMed, 11051354). Consequently, managing the properties of the gastric pool is a cornerstone of treating acid-related gastrointestinal disorders.
