Overview

Gastric lumenal hydrogen ions, primarily existing as hydrochloric acid (HCl), are secreted by the parietal cells of the stomach via the H+/K+ ATPase pump (StatPearls, 2023). These ions maintain a highly acidic environment (pH 1.5 to 3.5) that is essential for the activation of pepsinogen into pepsin and the denaturation of dietary proteins (PubChem, 2024). Furthermore, the low pH acts as a critical innate immune barrier by neutralizing ingested pathogens (NIH, 2023). In pathological conditions such as gastroesophageal reflux disease (GERD) or peptic ulcer disease, these ions cause mucosal damage and pain when they contact sensitive tissues. While drugs like proton pump inhibitors (PPIs) and H2-receptor antagonists reduce the secretion of these ions, antacids target the ions directly within the gastric lumen. Through a chemical neutralization reaction, antacids react with the hydrogen ions to form water and neutral salts, rapidly increasing the intragastric pH and providing symptomatic relief (StatPearls, 2023). This direct interaction makes gastric lumenal hydrogen ions a unique chemical target in clinical pharmacology.
