Gastric mucosal and ulcer crater proteins refer to the proteinaceous exudate, primarily composed of albumin and fibrinogen, that is exposed at the site of gastric and duodenal ulcers [1.1.1, 1.1.2]. These proteins carry a strong positive charge in the acidic environment of the stomach, making them a unique target for the cytoprotective agent sucralfate [1.1.2, 1.1.3]. Upon activation by gastric acid, sucralfate polymerizes into a negatively charged, viscous gel that electrostatically binds to these proteins, forming a stable, protective barrier known as an ulcer-adherent complex [1.1.2, 1.1.4]. This physical shield prevents the penetration of gastric acid, pepsin, and bile salts, thereby protecting the underlying damaged tissue from further erosion [1.1.1, 1.1.3]. Furthermore, this interaction helps sequester and concentrate endogenous growth factors at the site of injury and stimulates the local production of prostaglandins and bicarbonate, which collectively promote mucosal repair and epithelial regeneration [1.1.2, 1.1.5].
