Overview

Gastric mucosal glycoproteins are high-molecular-weight proteins, primarily represented by the mucins MUC5AC and MUC6, which serve as the fundamental structural components of the gastric mucus gel layer (Nordman, H., et al., 1995, Glycoconjugate Journal). These glycoproteins are characterized by extensive O-linked glycosylation, which enables the formation of a viscoelastic barrier that protects the underlying gastric epithelium from mechanical stress, hydrochloric acid, and proteolytic digestion by pepsin (Ho, S. B., et al., 1995, Cancer Research). In clinical pathology, the degradation or reduced secretion of these glycoproteins is a hallmark of peptic ulcer disease and Helicobacter pylori infection, where the pathogen often colonizes the MUC5AC-rich surface layer (StatPearls, 2023, Physiology, Stomach). Therapeutic strategies targeting these glycoproteins include cytoprotective agents like sucralfate, which binds to the glycoprotein-protein complex at ulcer sites to prevent further damage, and rebamipide, which increases the glycoprotein content to promote mucosal healing. Consequently, maintaining the integrity of these glycoproteins is essential for gastric homeostasis and the prevention of acid-peptic disorders.

Mechanism of action

Drugs interact with gastric mucosal glycoproteins by either physically binding to the glycoprotein-rich mucus layer to form a protective polyanionic barrier over ulcerated tissue or by stimulating the synthesis and secretion of these glycoproteins via prostaglandin-dependent or independent pathways to enhance the endogenous mucosal defense (Nagashima, R., 1981, Journal of Clinical Gastroenterology; Arakawa, T., et al., 1998, Digestive Diseases and Sciences).
