Overview

The gastric mucosal prostaglandin system is a complex physiological network essential for maintaining the integrity of the stomach lining against aggressive factors such as gastric acid and pepsin. It primarily involves the synthesis of prostaglandins, specifically PGE2 and PGI2, through the action of cyclooxygenase enzymes (COX-1 and COX-2) and their subsequent signaling via G protein-coupled receptors like the EP and IP receptors. These prostaglandins provide 'cytoprotection' by stimulating the secretion of protective mucus and bicarbonate, enhancing mucosal blood flow, and inhibiting gastric acid production. Clinical relevance is most prominent in the context of nonsteroidal anti-inflammatory drugs (NSAIDs), which inhibit this system and increase the risk of peptic ulcers and gastritis. Conversely, synthetic analogs such as misoprostol are used to mimic these protective effects, particularly in patients at high risk for NSAID-induced gastropathy.

Mechanism of action

Drugs modulate this system either by inhibiting cyclooxygenase enzymes (COX-1 and COX-2) to reduce prostaglandin synthesis, which can lead to mucosal injury (NSAIDs), or by acting as synthetic prostaglandin analogs (e.g., Misoprostol) that activate EP receptors to enhance mucosal defense and inhibit acid secretion.
